Chemical Health Assessor

JOB SUMMARY

The Chemical Health Assessor at JustUs Health assists individuals, with a focus on people living with HIV, in addressing chemical health concerns. This person provides chemical health counseling, Rule 25 assessments, reassessments, and assessment updates, placement into treatment programs, planning and coordination of aftercare services, and consultation for community providers. The Chemical Health Assessor also works with medical, social service, mental health, and chemical health service providers throughout the community to ensure high-quality services for all clients.

 

ESSENTIAL FUNCTIONS

  • Engage with clients, one-on-one to develop rapport and build relationships.
  • Complete chemical health assessments, updates, and treatment plans.
  • Provide placement coordination and follow-up to chemical dependency treatment programs.
  • Meet with clients and provide services on- and off-site to maximize client recruitment and retention.
  • Coordinate services with supporting professionals, including HIV service providers, medical professionals, and chemical dependency providers.
  • Provide chemical health counseling to clients in order to support their chemical health goals.
  • Maintain a thorough understanding of HIV disease, transmission, and services.
  • Deliver health information to clients in ways that support their health management and reduce the risk of HIV transmission.
  • Complete charting and billing in an accurate and timely manner in compliance with agency protocols, contract standards, and legal regulations.
  • Establish and maintain professional working relationships with other organizations and professionals.
     

QUALIFICATIONS

  • Bachelors Degree or equivalent work experience 
  • Licensed as an Alcohol and Drug Counselor (LADC) or be immediately eligible for LADC upon hire
  • At least one year of experience in providing chemical health services, including: chemical health assessments, reassessments, and assessment updates, interdisciplinary care coordination, resource development, development and utilization of client-centered care plans.
  • Skill and expertise working with African American, Latino, and multi-racial communities who are disproportionately impacted by HIV.
  • Comfort and skill in addressing gay/bi/MSM/queer and transgender health issues.
  • Capacity to work with HIV-positive people, and those engaged in risk behaviors for HIV, in individual and group settings.
  • Comfort working with the following topics from a client-centered, risk-reduction perspective: HIV health, chemical health, mental health, and sexual health.
  • Excellent verbal and written communication, problem-solving, and organizational skills.
  • Valid Minnesota Driver’s license, access to reliable transportation, and ability to travel independently throughout the Twin Cities Metro Area is required.

Preferred Qualifications:

  • 2-4 years of experience in maintaining a diverse, chemical health caseload.
  • 1-4 years of experience in working in the HIV services sector
  • Proficiency in a second language other than English.
  • Individuals with lived experience or a passion towards our target populations (Gay/Bi/MSM/Queer, Transgender, African American, multi-racial communities that are disproportionately impacted by HIV, HIV positive people, and those engaging in risk behaviors for HIV in individual and group settings) are encouraged to apply.

 

WORKING ENVIRONMENT

 

  • Travel to sites in car as needed – not to exceed 60% time out of office

  • Long-distance or air travel as needed – not to exceed 40% travel. 

  • Work outside of normal office hours may be required including nights and weeks

  • Periods of stress may occur.
